WASHINGTON, DC (TDR) — Medicare telehealth coverage received a temporary reprieve through January 30, 2026, but millions of elderly Americans face an uncertain future as Congress debates whether to make pandemic-era flexibilities permanent or allow pre-pandemic restrictions to return.

The temporary extension allows Medicare beneficiaries to receive telehealth services from home for routine medical care, but coverage will revert to requiring patients be in rural areas at approved facilities starting January 31 without congressional action. Mental health services remain permanently covered from home.

Elderly Face Significant Access Barriers

Healthcare advocates warn the potential loss of at-home telehealth would disproportionately affect elderly patients managing chronic conditions who have come to rely on virtual check-ins with their doctors.

“People with Medicare are accustomed to checking in with their health care providers from the comfort of their own home to assist with managing chronic conditions or see specialists that work far away,” AARP government affairs director Andrew Scholnick stated.

Research from Brown University found approximately fifteen percent of Medicare beneficiaries had telehealth appointments in early 2025, primarily for chronic conditions. Usage rates were highest among those with end-stage renal disease and long-term disabilities at thirty-seven percent.

Transportation and Mobility Concerns

Medical professionals emphasize that many elderly patients face significant challenges accessing in-person care. Transportation difficulties, mobility limitations, and vulnerability to infections in crowded waiting rooms make telehealth an essential option rather than mere convenience.

Dr. Nisha Mehta, founder of Physician’s Side Gigs representing nearly 200,000 physicians, explained that telehealth has become critical for providing access to patients who might otherwise fall through the cracks. She expressed concerns that seniors struggling with transportation or living in remote areas might forgo care entirely without telehealth.

Multiple bipartisan bills have been introduced to extend or make permanent the Medicare telehealth flexibilities. The Creating Opportunities Now for Necessary and Effective Care Technologies Act would eliminate expiration dates for expanded provisions, while other legislation proposes two-year extensions.

More than 450 organizations signed a letter urging congressional leaders to act on a long-term telehealth solution for patients, providers, and the healthcare system.

Cost and Quality Questions Remain

Some policymakers worry expanded telehealth could increase healthcare costs if virtual visits lead to additional in-person care. However, Kaiser Permanente research suggests telehealth visits can substitute for in-person visits, with return visits after video and phone appointments remaining uncommon.

Medicare Advantage plans, which are private plans contracting with Medicare, will continue offering expanded telehealth benefits after original Medicare restrictions return, creating a two-tier system based on plan type.

The American Medical Association and American Hospital Association continue advocating for permanent telehealth authority, calling short-term extensions unacceptable.
